The technical data sheet for the 401A04 contains the following voltage ranges:
+/-5V input range for correct operation
+/-10V overvoltage (no correct operation, but the device should not be damaged)
The device can be damaged by static discharge, so that could be a
possible explanation why some of you damaged their device although the
applied input voltage was within the allowed +/-5V range.
