The ordering of your elements in your clusters is incorrect. I'm not sure how you managed to do that. On the front panel, right-click on the clusters and select "Reorder Controls in Cluster...". You will see that the one for the output voltage is incorrectly ordered.

You are correct again, I though that the order was determined by the order the elements were inserted into the cluster but that doesn't seem to be the case.  I have this corrected and thanks again and I'll have to put that in my note so I don't get tripped up again.
