How to get a LV 5.1.1 cd replacement

Hello,
We are planning a possible hard drive/OS update for one of our testers running LV 5.1.1. Nobody can seem to locate the original installation cd. I see a license number in help/about of z00g14736. What is involved in getting a cd replacement?
