My custom icon does not appear in the task bar or in the top left hand corner

I am using LabVIEW 8.6 to generate an executable. I am using the 16x16, 4 bit colour icon. The shortcut is fine on the desktop, but it does not appear in the project view, it does not appear to the left of the application name in the task bar or in the top left hand corner of the front panel.

Previous posts have mentioned using 16x16 bit icons, but I am doing this already. Is there anything else that I might be doing wrong?

Is it included in your project file? Have you selected to use the icon when building the .exe? Check the icon in the project, it's best if it's made for all 3 color resolutions.

I had selected my icon and 16x16 for the Icon Image, but in the icon editor, I had added an icon for 32x32, but not one for 16x16. When I added the 16x16 version it suddenly appeared. Thanks for your help.
