PCI-8232 Not recognized in Intel Motherboard DH67CL

I am attemoting to set up an ETS on an Intel DH67CL motherboard.  I am using the PCI-8232 Ethernet card that comes with the ETS bundle.  The motherboard has 3 PCI slots and when I place the card in (slot numbering :

 

Slot 4:  On boot, does not make it through POST and gives the beep codes for not being able to find video.

 

Slot 3:  Same as Above

 

Slot 2:  Gets through boot, starts the LV USB Utility.  When I run "7. Evaluate System".  I get a error: 

"No supported network device found. A network adaptor with a supported ethernet chipset is required."
Because of the form factor of the computer, I will need to get the PCI-8232 working in Slot 4.
I have upgraded the BIOS to the latest available from Intel.
Bad card?  Incompatible BIOS?

Still can not get passed this. Can't boot into safe mode or format because USB boot can not find the network card.

With the card in slot 2 I can see a 32 bit card 33Mhz is present. Inside the BIOS diagnostics. Other slots, I don't even get past POST so it is impossible to tell.

I am not sure I follow. These look to be for installing the PCI-8232 in a desktop computer with Windows installed. I am installing the card in a desktop with no OS and setting it up as ETS.

I'm not familiar with ETS, it is like a real time os or a bootloader?

It is the real-time stand-alone.  I am essentially installing per:

I am configuring using a USB and then formating and installing the RTOS.

First I'd like to ask you, which processor you are working with?

The processor is one of those listed, an intel i-3 sandybridge (i3-2100). I am using LV2011. So it should be compatible.
