I have setup a test system over here with Labview 2011 SP1 and RIO 4.0 and the device apppeared in the Windows Device Manager. If it doesn't appear in Windows Device Manager, it will not appear in MAX. Go to the Windows Device Manager and this time select View >> By Connection. Then open all of the PCI-PCI bridges and see if you can find the device here. If you cannot, then Windows is not detecting the card at all.
Look for anything with a yellow bang. This indicates that Windows does detect it, however a driver has not been associated with the card yet. If you cannot find the card in Windows Device Manager, then I would think the card needs to be RMA'ed.
I don't think this is the issue since your other NI Cards seem to be fine, but you can go ahead and check services. Go to Start -> Run and then type in "services.msc". In the service window, scroll down to the NI section. Anything by default that has a startup type of "automatic" should be started in order to NI software/hardware to work properly.

Hi Jason,
I agree with you, that the card needs to be RMA'ed, and have returned PXIe-5641R to the corresponding service of our institute.
I will wait for result.
