adding custom icons in LV 5.1

In the app builder in LV 5.1, there is a tab for "App Settings." Here,
I am checking the custom icon box and browsing to my 32x32 16 color
icon. Once selected I get a message stating:

The selected file doesn't contain all of the needed icons. Without
them, your application will show an incorrect icon in some views.

How do I get all four icons to be accepted via this browse text line?
It appears to only let me select one *.ico file at a time. Any help
would be much appreciated.

Each .ico file should actually have 4 icons in it (two 32x32's and two
16x16's, with B&W and color for each size). Use an icon editor to make
sure all types are present. There are shareware/freeware ones at
www.download.com
