I follow the instructions given in MAX and the result is always "Operation Failed"

I have seen this problem when the database is corrupt.
Even the minor corruption on the database will keep the "Import 3.8 Database" utility from working.

I suggest you to contact the NI Tech Support to see if they can help you find the corruption.
