Lookout V6 Mailer Error

I am having trouble entering the data required for the Mailer object.  Am I missing something?   

I have attached a screen capture of the mailer object.

Looks like you are missing a whole bunch of quotes in the yellow fields, and also missing a return email address.

Is there an completed example of the mailer object located anywhere? I couldn't find one in help or online.
