Make Formated Report

Hi, my name is alejandro, i know that some one else ask something like that i am gona ask, but i read the answers, and do not solve my problem at all, i make an HTML Report but i want to give a format, it look like this...

Thank you for posting your question on our Discussion Forum. Are you using Lookout? If so which version? Are you using the 'Report' object in Lookout to generate the report? I would need some more information regarding your application and what exactly you are trying to do before I can give you some tips.
