How to connect UMI 7764 with servo driver

I'm using PCI-7344 & UMI 7764 to control ac motor on servo motor (Panasonic  MUDS5A5A1A),

but i have a question, the  voltage of UMI 7764 is 5V  while voltage of driver is from 12 to 24V. What should I do?In addition, I don't know how to connect UMI7764 with the driver . The attachment is the connection drawing.
