I tried calling and they said I need some type of service reference number.

Please tell them what kind of hardware you have and that you need to setup an RMA, the customer service rep will create a service request for you and get you in touch with our RMA department. Thanks

I am having the SAME problem AGAIN. The card worked when I got it back in April. Then I wasn't using it over the summer but it was still working. I came back and am setting up to record and now the card is not working again. The same exact errors in the diagnostics again. I need a card that works for more than a month! PLEASE HELP. I have attached the diagnostic file.
