Possible causes for PCI-MIO-16E-4 failure

I have a PCI-MIO-16E-4 and two days ago it stopped working. Basically the digital I/O channels would always give a 5V output even when forced manually (using the labview hardwere controller program) to give a 0V output.
Once I removed the card I noticed that the chip marked as 70266 2A-01 @ NI was partially melted. I have been using the same program for awhile and it never gave me any problem. The only difference is I changed computer (upgraded from Gateway to Dell) and I transferd all the softwere and the card to the new unit and at the third run of the program the card failed.
Now I am about to buy a new one but I would like to know if there is any documentation on this type of failure and what could be a probable cause for it to happen.

Hi Federico,

A melted chip is usually caused by too much voltage running through the card.  This can occur if you do not unplug the power to the PC before removing or inserting the card.  As long as you make sure of this, you should be okay with the new card.
