Why do I have extra entries in my MS hierarchy tab?

I have duplicate entries for schematic sheets in my hierachy tab of the design toolbox. 

Why would this happen?

How can I fix it? 

Does it even matter?

I've never see or heard of this before. Is it possible to post the file so I can take a look or do you know how you got your design into this state?

I don't know how it got that way.  I just noticed it a few days ago.  Tiem Pham has a copy of the file you could look at.  He has been wortking on it for other reasons.  I cannot post it publicly.

When I checked the files today the extra sheets are no longer there.  Must have healed itself by a re-boot or re-start or something.

My bad, the one I checked again was not actually the file that had the problem.  The problem is still there in the actual file that had the problem.  I can send this to tech support (after I double check it again).
