PCI 8433/2 showing up as PXI card in device manager and NIMAX

I have a PCI 8433/2 showing up as PXI card in device manager and NIMAX.

The PXI number varies randomly. 

After power-cycling the PC earlier the card did briefly display the correct type, but then reverted to a random PXI card ID on next boot up. 

Is this likely the card has failed, or is it a driver issue?
