I've got a couple of things for you to try:
Swap the card to the other PCI slots on the computer and see if the problem remains.
Install the driver on another computer and test whether the test panel works now.
If both these fail, you may have a faulty card, which will need repairing. Contact National Instruments directly, you will be taken through troubleshooting steps and if required advised about how to return the card for replacement or repair.
