MID-7604 - only see 2 axis

I am using the MID-7604 with PCI card adapeter.  In MAX, I can see axis1 and axis2 (and have control over both).  However, I do not see axis3 or axis4.  The 7604 should support 4 axis.  What am I doing wrong?
