It seems to be specific to this VI. I don't have another PDA to be able to test this. I tried deleting everything, except the quit button and it seems to crop up anyway. Once it has shown up, it doesn't appear to go away. So what I tried doing was loading from a backup, (i.e. hadn't added the graph and what not). About the refreshing issue, I tried opening up something and closing it and then looking at the VI and the black square had gone away. BUt, when i deleted everything and had only the stop button, even if i opened something up, it would not go away. I am not sure to why this is happening.
