My CLAD exam was $129 this past October, in Los Angeles. For that one, you have to create an account with Pierson VUE to register and see the price, schedule, and location.
The higher level exams are schedule through NI, and you should be able to see the prices on-line.

For CLAD and the recertification exams, you register with PearsonVUE (http://www.vue.com/ni/). For CLD and CLA, you need to arrange for a proctor (e.g., your DSM).

The ones I have taken that weren't the actually programming (CLD), the initial CLAD and the CLD recerts, are computer, multiple choice, time limited. You need to have official, government issued picture ID (I used my passport), don't remember if two forms were required, and I was almost strip searched, well had to empty everything from pockets, leave watch, before entering the testing area, which was under video survellience! Felt safe to fly again, I mean wire again!
