Why do the shortcut menus I create dynamically not fire events?

In the attached VI I create both application menu items and shortcut menu items both statically and dynamically on both the top level and sublevel (eight combinations in all).  All the menu items should fire an event and populate the ItemTag string with their ItemTag.  They all work except for the dynamically created ones on the shortcut menu (Shortcut 2 and Shortcut 4).  Why don't these cause an event to be fired?

Make sure you get the edited one, I attached the wrong thing the first time.

Also, shortcut 4 isn't showing up because you have the wrong menu tag. You had "shortcut to shortcut 2" or something of that nature.
