detect abnormal case with ROI

Hi everyone.

I have several images to process using threshold function and I would like to automatically detect the abnormal condition.

Can anyone help me to construct the coding for ROI function?

A circle in the picture that I've attached is the example of abnormal case that I would like to inspect.
