PCI-6221 DAQ card is suddenly Outputing AND Inputing 0.4V high

It happend 1 week ago and i cannot fix it.  there is no way to calibrate to output.  i tried the self cal vi.  its a linear 0.4V offset from 0 to 10V.

i just need a pin-out to tie the neg- to ground.  there is only a pin out for the input, but no ground indication

You can find device pinouts on Figure A-2. PCI/PXI-6221 (68-Pin) Pinout of the M Series User Manual. you can pin to AO ground at terminals 21 & 22.
