A) I'm also not able to reproduce it with your simulation settings.

If you find a fixed way to reproduce it, then please let me know so that I can report the behavior.

I think the error got fixed by using the "set initial conditions to zero"-setting (why isn't that set default?) in combination with the Convergence Assistant that did this:"-Shunt resistance from analog nodes to ground (RSHUNT) was changed to 100000000", no idea what the previous/default value of that setting was.
