DC stabilized amplifier simulation issue

Hi

I'm trying to use Multisim 11 to simulate the circuit shown on page 10 of Linear Technology Application Note 21

When I do this the output of the LT1028 is pegged to the +ve supply rail.

Also the voltages on the supply pins of the LT1052 are nearly at the +/-15V supply rails rather than being +/- 5V (I can understand this one I think).

I'm guessing that the model for the LT1028 doesn't allow for the offset pins?
