custom UUT Information dialog box

hi. i have a sequence running in parallel model and i would like to customize the uut information dialog box to include some custom booleans i need for my sequence. Is there some kind of labview example code i can use to learn how to do that? 

doing it in sequential model seems pretty easy but i have no idea on how to manage the 2 sockets while keeping only one dialog box
