Open up a Windows Explorer window and goto Tools->Folder Options or go to Control Panel->Folder Options. Find the CHM extension:
Click on the Advanced Button.
Select the open action and then click the Edit button.
Check to see if the settings match mine.

There are known infection of hh.exe, so maybe it is infected and your antivirus blocks access to it. Check your AV log or system log for interesting entries.
Maybe you should submit your hh.exe to an online virus scanner, such as Virustotal.

I found a fix for the problem at the following link

The HHCTRL.ocx file got messed up somehow
