 Here is the sample code.  It works fine except that when i click cancel, it gives me the error.

You have some tunnels set to Use default if unwired.  And you don't have wires such as the notifier reference wire run through every event case or every case structure case.  You can see that wherever there is a tunnel that is not solid.
