Problems importing APT database into Tiway object

I have been unable to successfully import an APT tag file into a Tiway driver object in any version of Lookout. I receive an error message saying "Not an APT tag file!". I have tried multiple versions of Lookout and multiple install.tag files. Any suggestions?

There is some type of file header or standard infromation the APT file should use to identify itself to Lookout. What do you use to create the APT tag file? Could there be an option within that realm that could be preventing Lookout from thinking it is a valid file?

My files contain what appears to be a standard file header, in a csv-type format. There are not any options in creating this file from APT, only to create it. The following is the file header and first data points in my file:
