Dipswitch erorr

INestor,I thought I would bring this to your attention. I do not know if this issue has previously been discussed or not. If it has and your already aware of it then accept my apology.
 
It seems like the DSWPK_5 Dipswitch is malfuntioning and returning errors. Below I have posted my simple test circut  you take a look at. I tested the rest of them and they are O.K.. Just this is is causing problems.
